Full Time: 30-40 hours per week (0.75 or 1.0 FTE)
Position Summary: Provide oversite regarding medication monitoring and medical consultation and training at all housing programs.

Position Summary: Provide oversite regarding medication monitoring and medical consultation and training at all housing programs.
Position Responsibilities:
- Provide medical consultation, including crisis consultation, and act as liaison between client/program and medical facilities or providers.
- Evaluate client’s potential for admission (disabled, high acuity, etc.).
- Provide emergency first aid and refer to 911/hospital when needed.
- Provide follow-up on all hospitalized clients.
- Coordinate admits/transfers from Mission Nursing Home.
- Ensure that medical equipment is in proper working order.
- Assist residents with COVID testing as needed.
- Assist residents with blood glucose testing, as needed
- Provide blood pressure monitoring.
- Conduct medical history assessments of all new residents and make referrals as necessary.
- Oversee medication monitoring including destruction of discarded/unused/outdated medications.
- Train staff on medication monitoring.
- Conduct program/client-specific monthly education classes.
- Assist clients with making medical/physical therapy appointments
- Review and provide input on all medical related policies and procedures.
- Conduct Narcan and other staff training twice monthly.
- Key participant in Quality Improvement program including review of quarterly reports and meetings to ensure quality of medical response and monitoring is provided.
- Review and provide input on Infection Control Program plan, policies and procedures changes or updates, as needed.
- Do Mantoux testing on all new employees except JRC.
- Other duties as required.
- LPN or RN
- Knowledge of addiction and alcoholism and medical complications associated with both.
- Two years’ experience in addiction medicine preferred.
- Ability to work well with people from diverse racial, cultural, social, and economic backgrounds.
- Ability to meet the physical demands of the job which may include walking, bending, stooping and kneeling multiple times per shift.
- Ability to pass a criminal background check.
- Lifting up to 25 pounds..
- Ability to ascend and descend at least two flights of stairs.
